Veteran Banner Order Form

Please carefully fill out the information and Indicate the correct branch/Version of the banner that you wish to choose. You must also provide a photo that will be used on your Veteran’s banner. Pictures will be returned. The banners will be hung for Memorial Day and Veteran’s Day for two years for the cost of $50. After two years, you can re-purchase a banner to hang to keep the retired one.
Have this order form turned In by Oct 1, 2024, for this year’s Veteran’s Day.
